Information on Procedures for Requesting Personal Information Subject to Disclosure

We handle requests for disclosure, correction, addition or deletion of details, suspension of use or removal of information, and suspension of provision to third parties (hereinafter “requests for disclosure”) of personal information subject to the completion of our designated request form. You may download a PDF copy of the designated request form at our website.

1. Address for mailing “Request for Disclosure”

To make a request for disclosure, fill out all designated items on the request form (1), and mail it with the required documents (2).

2. Documents to be submitted with request for disclosure

To make a request for disclosure, fill out all designated items on the request form (1), and mail it with the required documents (2).

    1. (1) Designated request form
    1. “Request for Disclosure of Personal Information Subject to Disclosure”
    1. (2) Documents for identity verification

Include a copy of one of the following forms of ID:
① Drivers license
② Passport
③ Resident’s or Residency card
④ Health insurance card
⑤ Other official government document to verify identity
*If your domicile of origin or government ID number is included in the document, please cross it out with a black marker.

3. Request for disclosure by proxy

If the person making the request for disclosure is a proxy, in addition to the above documents listed in (1) and (2), include a copy of one of the documents listed below to verify your proxy status.

Documents to verify proxy status
(1) If you are a proxy for the purpose of requesting disclosure
– Letter of proxy from the individual (original)
(2) If you are the legal representative of a minor
– Copy of family register
– Other official document for verifying authority of statutory representation
(3) If you are the legal representative of an adult ward
– Certificate of registration concerning guardianship
– Other official document for verifying authority of statutory representation

4. Fee for request for disclosure and method of collection

We will charge a fee of 1,000 yen in the case of request for notification of purpose of use or disclosure. Please enclose a postal money order for that amount with the request form.

5. Method of response to request for disclosure

In principle, we will respond in writing (sealed letter) to the address of the individual listed on the request form.
◇ Personal information obtained in conjunction with the request for disclosure will be handled within the scope necessary for responding to said request.
◇ We may be unable to respond to certain requests for disclosure (see below). In such cases, we will provide notification of the reason(s).

Please note that the 1,000 yen fee is charged even if we are unable to provide the requested disclosure.

      1. We are unable to verify the identity of the individual or proxy
      2. Certain designated documents are missing or certain information on designated documents is missing
      3. The information from the request for disclosure does not fall under the category of personal information subject to disclosure*
      4. There is a risk to the life, health, property or other rights and interests of the individual or a third party
      5. Doing so could greatly impede the proper conducting of our business
      6. Doing so would violate the law

*Personal information subject to disclosure refers to personal information systematically arranged for which we have the authority to respond to all requests for disclosure, correction, addition or deletion of details, suspension of use or removal of information, or suspension of provision to third parties. However, the cases described below do not fall under the category of personal information subject to disclosure.

a) The existence of the personal information becoming known would put the life, health or property of the individual or a third party at risk.
b) The existence of the personal information becoming known could further or give rise to unlawful or inappropriate acts.
c) The existence of the personal information becoming known could put national security at risk, damage the trust of other countries or international organizations or create a disadvantage in negotiations with other countries or international organizations.
d) The existence of the personal information becoming known could hinder the prevention, suppression or investigation of a crime or the maintenance of public safety and order.
